At Kittitas Valley Healthcare, we welcome patient and visitor feedback on our service. We look forward to helping you with your concerns, or hearing ideas that you may have for improving our service.
We are especially interested in hearing of any safety concerns or suggested improvements, as well as examples of exceptional care that you received from our staff.
How You Can Let Us Know
While verbal compliments or concerns can be expressed to staff or management, you can also communicate your compliments or file a complaint by filling out a Care and Service Report. This form allows you to write down, in your own words, any comments, questions or concerns you would like us to be aware of or to address. If you have trouble with the form, please call us for assistance.
After you complete the form, you can bring it to Quality Assurance, or submit it by mail or fax. You can also deliver the completed form to the hospital front desk.
We ask that you submit your compliment or concern by mail, by fax, by phone, or in person so that we can ensure your compliment or concern is kept secure. Electronic communication like email and web forms may not maintain your confidentiality.
What Happens Next
Our goal is to contact you by phone or in writing within 7 business days to let you know that we have received your report. From there we will attempt to address your concerns or help answer your questions.

You also have the right to contact the Washington State Department of Health at (800) 633-6828. Patients covered by Medicare may contact KEPRO at (888) 305-6759.
