Requesting Medical Records (Not including images)
WA State law allows up to 15 business days to respond to a medical records request. KVH staff will review same day requests to determine if the request can be completed same day. KVH generally provides records to patients and other healthcare facilities at no charge.
The easiest and quickest way to obtain copies of medical records is to fill out our online request form below. If you are not requesting your own records, please review the information below about requesting records for a minor patient, a deceased patient, or as a legal representative.

You can request your medical records securely clicking HERE (Spanish available). By submitting your request online, you have the option to have copies of medical records sent to you through a secure email link. Other options include faxing and US Postal Mail.

If you need a Portal account created for you, please contact your clinic directly. Please note: Portal access is not available for parents of minors between the ages of 13 to 18 years.
To request copies of diagnostic images (x-ray, CT, MRI, etc.), please call KVH Imaging Services directly at 509.933.8640.
Patients may find inaccuracies in their medical records. If this happens, you may request an amendment to your record. Call 509-962-7307 to speak to our friendly HIM staff about making an amendment to your record.

You can request birth and death certificates online from the Washington Department of Health.
You can obtain a copy of your sickle cell testing records online from the Washington Department of Health.
