Patient records at KVH Hospital are managed by Health Information Management (HIM).
Medical Records Request
Someone may need their medical records for various reasons, like sharing them with other providers. You can maintain your records on a Patient Health Record Application (digital platform) and/or keep paper records.
You may request your medical record securely on the KVH Website by clicking HERE (Spanish available). You will be asked to provide some information and your records will be sent to you through a secure link. Most records are available within 24 hours. KVH generally provides records to patients at no charge.

Images are available directly through the KVH Imaging Department. KVH Imaging has special equipment available so they can provide you a CD of your images.

Access your Family’s Immunization Information / Washington State Department of Health
MyIR allows you to manage your family’s immunization records securely online. Once you register, you can access the records any time you need them. Go to wa.myir.net/register to begin the sign-up process.
You can also print your child’s Certificate of Immunization Status (PDF) for school and child care entry. You can do all of this without an extra trip to your healthcare provider or school.
