
All patients are admitted to KVH Hospital by arrangement of a physician. This can be through a planned admission process or through the Emergency Department. Either way, patients will be requested to provide information about themselves, their insurance carriers, and their medical condition.
Preparing for a hospital stay
Please bring
- A robe, gown or pajamas, and slippers
- Toilet articles (comb, brush, razor, toothbrush and toothpaste, etc.)
- Reading materials
- A few dollars for personal items (newspapers, etc.)
- Glasses, hearing aids, dentures
- Advance directives (An advance directive is a form that indicates your medical and mental health treatment choices if you are unable to communicate. Examples are living wills and powers of attorney. If you have completed an advance directive, please bring it with you and it will be placed in your medical record.)
- Physician Order for Life Sustaining Treatment (POLST) forms (POLST is a directive that lets us know your wishes about care in an emergency situation, particularly if you have chronic serious medical conditions.)
- Any orders, records and physical forms your doctor may have given you
- Insurance information, including ID cards, forms, your Medicare number; pre-authorization letter; L&I claim number and/or DSHS coupons
- Any medications you are currently taking, and a list of the medications and the dosages taken daily
- Emergency telephone numbers of family or friends, if they should need to be contacted
Please don’t bring
- More than $10 in cash
- Credit cards, jewelry, and other valuables
- Hair dryers, radios, televisions
