The Geriatric Nurse Practitioner (GNP) program provides medical assessments and management of acute or chronic illnesses in skilled nursing facilities, assisted living facilities, adult family homes, and for home-bound patients in Kittitas County. The GNP program works in collaboration with the patient's primary care provider.

The role of the GNP includes ongoing alliance and communication with physicians, family, staff, and community agencies for special care needs and continuity of care.

GNP services include
  • Completing admission assessments to nursing facilities as needed.
  • Managing day-to-day acute or chronic conditions.
  • Ordering diagnostic tests to diagnose and treat.
  • Communicating with the patient's family or facility staff as indicated.
  • Collaborating with the primary physician to report medical findings and follow up, and following orders given by the physician.
  • Obtaining lab specimens. 
  • Teaching caregivers/family members how to care for the patient.
  • Managing medications to alleviate symptoms, pain, or to manage acute or chronic conditions.

The GNP program operates through KVH Family Medicine - Ellensburg. KVH Family Medicine - Ellensburg will bill any insurance that you have provided to the program. The statement you receive with your balance will have KVH Family Medicine - Ellensburg's facility name on it. Please call 509.933.8657 with any billing questions.

How can I request this service?

The GNP service is available to residents living in skilled nursing facilities, assisted living facilities, and adult family homes.  The GNP service is also available to home-bound patients in Kittitas County. If you have not been referred to this service but would like to be, please call us at 509-925-8483.

How often will I be seen by a GNP?

When you are in need of a health care provider at your facility, staff will inform the GNP. Monthly reviews will also be done to reconcile medication, address any additional concerns, and assist providers in delivering quality health care. Updates are given to family members by staff of the skilled nursing facilities or assisted living facilities. The GNPs visit these facilities weekly.  If the patient is home-bound, the GNP will work with the patient and the provider to establish an appropriate schedule.
