Life and accidental death and dismemberment insurance (AD&D)
Provided through Cigna. KVH pays all premiums for full-time employees; premiums for part-time employees are prorated. Employees with a 0.6 FTE or greater are eligible to participate in life and AD&D insurance. Per diem employees are not eligible for life or AD&D. The benefit amount is $25,000 for life insurance and $25,000 for AD&D. Employees may elect additional voluntary life or AD&D and are responsible for those premiums.
Long term disability insurance
Provided through Cigna. KVH pays the premium for all employees 30 hours (0.75 FTE) per week or more. Long term disability insurance takes effect on the first day of the month following six months of employment. The base plan provides 60% of the last month's salary up to a maximum monthly benefit of $6,600 after a 90-day waiting period. LTD does not apply to part time employees with an FTE under 0.75 or per diem employees.
Employee assistance program (EAP)
Provided through Kepro. KVH employees and their eligible family members (any family members residing in the household) are eligible to receive EAP services. The EAP is a confidential assessment and referral service specially designed to assist employees and their families in handling personal problems.
Voluntary insurance options
Kittitas Valley Healthcare offers voluntary short term disability, cancer care, accident and critical illness insurance through Allstate and AFLAC.
